Investigation by Aircraft Accident Investigation Commission Completed

The Commission for the investigation of the Vl-3E-1 Revolution ultralight aircraft accident cause, which took place on 23 July 2022 on the Nišići plateau, which was appointed by the decision of the Minister of Communications and Transport of Bosnia and Herzegovina, completed the investigation and made the Final Accident Report.

The entire procedure of investigating the cause of the accident was carried out in line with international standards and procedures of the International Civil Aviation Organization (ICAO) and the applicable legislation of Bosnia and Herzegovina.

In accordance with the "Sarajevo Tour" aviation event plan, a group consisting of 10 aircrafts (seven gyrocopters and three ultralight aircrafts), performed a previously-announced and approved group flight from Becsehely Airport to Pécs Airport (LHPP), Hungary, and then continued the flight to the airport Tuzla (LQTZ) and Nišići Airport (LQNI), Bosnia and Herzegovina. The flight was conducted at an altitude of 1000ft AGL, according to VFR rules and VMC conditions. The plane VL-3E, call sign F-JVXJ was the last (tenth) aircraft in the group.

During the flight in the area of the Nišići Airport, at a distance of approximately 650 meters east of the PSS, the pilot of the Vl-3E airplane suffered a stroke, after which he lost control of the airplane. The airplane fell into an irregular position (stalling) and in an uncontrolled flight at a steep dive angle, it hit the ground. The plane was destroyed, and the pilot and the passenger were killed.

Based on the investigation conducted, analysis of collected evidence and available information about the accident, the Commission drew the conclusion that the main cause of the accident was the plane falling into an incorrect position due to loss of airplane control as a consequence of the stroke suffered by the pilot.

The causes of the airplane accident are as follows:

a)      Immense psychological and physical strain, fatigue and probably stress of the pilot,

b)      Probable partial or complete engine failure due to lack of fuel in the area of the accident site, which mostly contributed to the condition faced by the pilot,

c)      Inadequate organisation and preparation for flying,

d)      Neglecting the aircraft technical correctness during the flight preparation, in particular the amount of fuel,

e)      Flight conditions: insufficient flying experience and group flying, high outside temperature, high daily stress of the pilot, etc.,

f)       Incorrect calculation and control of in-flight fuel consumption and reserves,

g)      Insufficient knowledge of the airport area and flying conditions.
